New York ACE 2006 registration fees include the general sessions,
all workshops, and the ACEing the Interventional Cardiology
Board Exam workshop. Also included are the Welcome
Receptions, continental breakfasts, lunches, course syllabus,
and CME certificate. Please be sure to check the Workshop and
Sunday Evening Reception registration boxes if you plan to attend.
Workshop registrations are limited, and space will be awarded on
a first-come, first-served basis.

Visa Requirements and Letter of Invitation
If you require a visa to enter the United States, please check with
the American Embassy or Consulate in your home country to
obtain the required documents. Additional information can be
found at www.dhs.gov/us-visit.
On request, the New York ACE 2006 Program Office is pleased to
provide an invitation for participation in this educational activity.
Such an invitation is not a commitment on behalf of Lenox Hill
Hospital, or any endorsing organization, to provide financial

Cancellation Policy
Your full New York ACE 2006 Meeting tuition is refundable if
your cancellation is received, in writing, prior to Friday,
September 29. After September 29, 2006 the tuition is refundable,
less a $100 processing fee, upon receipt of your written cancellation.
No refunds will be given after Friday, October 6, 2006.

Lenox Hill Hospital reserves the right to modify the activity content
and faculty, and associated activities, and reserves the right to
cancel this program if necessary. In the event of cancellation, the
full tuition will be refunded. We are unable to refund flight and
other travel-related expenses such as hotel charges.

Hotel Information
The Grand Hyatt New York will serve as the New York ACE
meeting headquarters. The hotel is conveniently located in
midtown Manhattan and is attached to Grand Central Station.
It is just a short stroll from Times Square, Broadway theatres,
5th Avenue shopping, Rockefeller
Center, and the Empire
State Building.
 New York ACE has negotiated a discounted room rate with the Grand Hyatt. Please make
reservations directly with the hotel, and be sure to make
them early, as fall is peak travel season in New York City. A block of rooms is
being held for New York ACE attendees until September 10, 2006. After that date, reservations will be
based on space availability, and the special room rate may not be honored.
